Once downloaded you can use this going forward. No need to download each request.
You can rename the file if you wish or save as if you need records of your requests. Do not change the format within the file
Must stay xlsx excel- DO NOT SAVE AS PDF or any other file
Only use the attached template - This has a table that will automate the flow when entered to other programs. Request Type and ProductNum is required in the file.
